What happened at the Jwaneng mine early Friday morning for a mine with such an impeccable safety record to lose a worker to such a seemingly routine and preventable accident? This is the question in the minds of many as the diamond mining giant, Debswana, counts the cost of last week's accident.

The company lost a foreman, Motshwari Raseiteo. His car was buried under tonnes of rubble when a pit wall collapsed. Raseiteo's car was only recovered on Saturday morning. Fifteen other employees were rescued on Friday morning, sent to Jwaneng hospital and released after treatment for shock and minor bruises.
